Photo ID required at all appointments.
You may also need to bring the following:
- Insurance/Medicaid card (if you have one)
- Children’s immunizations/TB skin test, for those 18 and under a parent/legal guardian must be present.
- Immunization appointments, if you have had shots at another facility please bring immunization records.
- Adoption/Foster Parents must bring court custody papers/placement papers.
- Sliding Scale, you must bring proof of income from all members in household.
Accepted proofs of income:
- W-2 form
- Most current pay stub
- Last year's tax statement
- Disability award letter
- Social security award letter
- Unemployment
- Child support
