Disease Investigation

To report a Public Health Emergency, call 620-259-2188 or email the Health Department. If this is a medical emergency, call 911.


The following diseases shall be designated as infectious or contagious in their nature, and cases or suspect cases shall be reported within seven days, unless otherwise specified, in accordance with K.S.A. 65-118 and K.S.A. 65-128, and amendments thereto: List of Reportable Diseases


To report a disease to the KDHE Bureau of Epidemiology and Public Health Informatics: KDHE Reportable Disease Form


For medical providers to report a disease to the Reno County Health Department, please complete the following form and fax it to our Epidemiology Program: Public Reportable Disease Form (PDF)


To have our Epidemiology Program follow up with you about a suspected disease, please complete: Public Reportable Disease Form (online)
